You can download the various documents above. Some can be filled out online, while others will need to be printed and completed by hand. This is intended to simplify the administrative procedures when you arrive at the practice.
Please complete the registration form in either French or English and print it. Don’t forget to carefully read the data protection document before signing your registration.
For the other documents, the reception staff will guide you through the process.
Once your sessions are completed, we invite you to fill out the satisfaction questionnaire and send it to us by email at secretariat@therapiedelatourelle.ch.
